Use this Incident and Injury Reporting form to report all accidents/incidents and near misses, whether an injury occurred or not and to document the investigation into the incident.

It is vital for Incident and Injury Report Forms to be filled immediately after the incident has occurred to ensure the event is recounted accurately.
